1. Peace Remedial Massage collects your personal health information directly from you, or from the person acting on your behalf. The information we collect may include:

• your name

• date of birth

• address

• health history

• records of your visits to us

• details of the treatment that you received

• We may sometimes collect personal health information about you from other sources, if we have obtained your consent to do so.

2. The purpose of collecting your personal information is to serve you in the best possible way. I will only use your personal information in ways that you might expect it to be used. I will not pass your personal information to any third parties without your permission, unless required to by law.

3. Your information is collected in digital form which will be secured by Clinico Practice Management System.

4. Upon request, I will give you the chance to see any information I hold about you.

 

5. If you believe there has been a privacy breach, please contact me through the contact details. I will contact you back ASAP.

 

6. When your personal information is no longer needed, I will take reasonable steps to destroy your personal information. However, most of the personal information is or will be stored in client files which will be kept by me for a minimum of 3 years.
